712ae73581 Kernel: Expose per-thread information in /proc/all
Previously it was not possible to see what each thread in a process was
up to, or how much CPU it was consuming. This patch fixes that.

SystemMonitor and "top" now show threads instead of just processes.
"ps" is gonna need some more fixing, but it at least builds for now.

Fixes #66.

35c26a06fc LibCore: Move puff() from LibDraw to LibCore
Since it's used both by CGzip and PNGLoader, it seems most appropriate
to keep this in LibCore.

f61ed8eab5 WindowServer: Listen for muted state changes from AudioServer
Now the menu audio icon updates between muted/unmuted when anything in
the system changes the muted state. :^)

9009390f9c WindowServer: Add an audio icon to the menu bar
Clicking on this icon toggles the AudioServer muted state.

It currently does not react to muted state changes caused by other
programs, since it has no way of learning about those from AudioServer,
other than performing a synchronous IPC call (GetMuted), which we don't
want to be doing in the WindowServer :^)

2b9ec22576 WindowServer: Optimize backing store placement for resizing windows
When a window is being resized, its size may differ from the size of its backing
store. In this case, peek at the direction the window is being resized in, and
render the backing store at the same place as it was previously.

555b4236f8 WindowServer: Ensure resized windows move in the expected direction
When a window is being interactively resized, there are several rules
beyond the actual mouse movement that can impact the new size of the
window, such as its size increments and minimum size limit.

Move the placement logic after applying all the sizing logic, so that
whatever final size the window ends up with, the sides of the window
that do move are the ones that the user is dragging.

69ca9cfd78 LibPthread: Start working on a POSIX threading library
This patch adds pthread_create() and pthread_exit(), which currently
simply wrap our existing create_thread() and exit_thread() syscalls.

LibThread is also ported to using LibPthread.

bf012ca10a WindowServer: Fix leaking framebuffer fd to clients
Now that the WindowServer's menu is the primary way to start apps,
it is especially noticable that it leaks the /dev/fb0 fd to the apps
that it spawns. Fix that by opening it with O_CLOEXEC.

dd2900eda0 Launcher: Remove the Launcher app, and all hacks in support of it
The Launcher's functionality has been replaced by the app shortcuts in
the system menu.

There were various window management hacks to ensure that the launcher
stayed below all other windows while also being movable, etc.

2fcf156540 WindowServer: Don't send menu item activations for separators
Separators can't have identifiers associated with them, so it's not
possible to react meaningfully to their activation. Don't send messages
about it to avoid confusing the clients.

5e61fd0e67 WindowManager: Simplify menu bar open/close logic
Let the global menu bar be either "open" or "closed". Clicking on one
of the menus in the menu bar toggles the state.

This ends up simpler and more intuitive than what we had before.

cbecad0a77 WindowManager: Move more of the menu management logic to WSMenuManager
This patch moves a whole lot of the menu logic from WSWindowManager to
its proper home in WSMenuManager.

We also get rid of the "close_current_menu()" concept which was easily
confused in the presence of submenus. All operations should now be
aware of the menu stack instead. (The concept of a single, current menu
made a lot more sense when there were no nested menus.)

74be54cce8 WindowServer: Organize system menu app shortcuts into categories
If the .af file for an app contains the App/Category key, we'll now put
it in a submenu of the system menu, together with all the other apps in
that same category. This is pretty neat! :^)

b6a6f34caa WindowServer: Populate system menu with app launchers from /res/apps
The new system directory /res/apps now contains ".af" files describing
applications (name, category, executable path, and icon.)
These are used to populate the system menu with application shortcuts.

This will replace the Launcher app. :^)

704f48d7f3 POSIX compliance: (most) shell scripts converted to generic shell
Ports/.port_include.sh, Toolchain/BuildIt.sh, Toolchain/UseIt.sh
have been left largely untouched due to use of Bash-exclusive
functions and variables such as $BASH_SOURCE, pushd and popd.

d0c11bbcc3 WindowServer: Fixed Menubar resize issue
The menubar bar wasn't being resized correctly, as the underlying Window
was never being resized when `DisplayProperties` was chaning the
resolution while the system was running. `m_window` now gets the
correct window size when it's been updated, so a reboot isn't required.

96f10c8de2 LibDraw: Rename Painter::blit_tiled() => draw_tiled_bitmap()
Also change the API to take a destination rect instead of a source rect
since internally it was basically creating a destination rect from the
source rect anyway. It was a little confusing.

bc319d9e88 LibCore: Make CObject reference-counted
Okay, I've spent a whole day on this now, and it finally kinda works!
With this patch, CObject and all of its derived classes are reference
counted instead of tree-owned.

The previous, Qt-like model was nice and familiar, but ultimately also
outdated and difficult to reason about.

CObject-derived types should now be stored in RefPtr/NonnullRefPtr and
each class can be constructed using the forwarding construct() helper:

    auto widget = GWidget::construct(parent_widget);

Note that construct() simply forwards all arguments to an existing
constructor. It is inserted into each class by the C_OBJECT macro,
see CObject.h to understand how that works.

CObject::delete_later() disappears in this patch, as there is no longer
a single logical owner of a CObject.

fcc3745b02 LibCore+LibGUI+WindowServer: Make events bubble up through ancestors
With this patch, CEvents no longer stop at the target object, but will
bubble up the ancestor chain as long as CEvent::is_accepted() is false.

To the set accepted flag, call CEvent::accept().
To clear the accepted flag, call CEvent::ignore().

Events start out in the accepted state, so if you want them to bubble
up, you have to call ignore() on them.

Using this mechanism, we now ignore non-tabbing keydown events in
GWidget, causing them to bubble up through the widget's ancestors. :^)

270adea22f WindowServer: Remove the default background wallpaper
Booting without a wallpaper is significantly faster in QEMU when the
host machine is under load (e.g while recording the screen..)

Using a wallpaper is now optional. :^)

d92e26d023 WindowServer+LibGUI: Allow switching windows in/out of fullscreen mode
You can now call GWindow::set_fullscreen(bool) and it will go in or out
of fullscreen mode.

WindowServer will also remember the previous window rect when switching
to fullscreen, and restore it when switching back. :^)

Compare the distance travelled squared against the max distance squared
to avoid using sqrt().

423807d772 WindowServer: Don't transform quick far-apart clicks into double-clicks
We now require that the two clicks that make up a double-click be no
more than 4px apart.

This fixes the annoying behavior where you'd often get incorrect
double-click events on GUI widgets.

ecbc0322c1 Applications: Create a display properties manager
An interactive application to modify the current display settings, such as
the current wallpaper as well as the screen resolution. Currently we're
adding the resolutions ourselves, because there's currently no way to
detect was resolutions the current display adapter supports (or at least
I can't see one... Maybe VBE does and I'm stupid). It even comes with
a very nice template'd `ItemList` that can support a vector of any type,
which makes life much simpler.

73fdbba59c AK: Rename <AK/AKString.h> to <AK/String.h>
This was a workaround to be able to build on case-insensitive file
systems where it might get confused about <string.h> vs <String.h>.

Let's just not support building that way, so String.h can have an
objectively nicer name. :^)

63e6b09816 WindowServer+LibGUI: Add support for nested menus
It's now possible to add a GMenu as a submenu of another GMenu.
Simply use the GMenu::add_submenu(NonnullOwnPtr<GMenu>) API :^)

The WindowServer now keeps track of a stack of open menus rather than
just one "current menu". This code needs a bit more work, but the basic
functionality is now here!

It's a little ladybug. Maybe someday we'll have a fancy icon, but until
then, this ladybug character is a cute placeholder. :^)
