5217875f6a Kernel: Consolidate API for creating AnonymousVMObject with given pages
We don't need to have a dedicated API for creating a VMObject with a
single page, the multi-page API option works in all cases.

Also make the API take a Span<NonnullRefPtr<PhysicalPage>> instead of
a NonnullRefPtrVector<PhysicalPage>.

9b14a8605a Kernel: Add a VERIFY() to make sure our DMA address is valid
This checks whether the address we're trying to use for DMA is low
enough so as not to overflow the I/O register.

b91df26d4a Kernel/Interrupts: Return boolean on whether we handled the interrupt
If we are in a shared interrupt handler, the called handlers might
indicate it was not their interrupt, so we should not increment the
call counter of these handlers.

a324d4d6a3 Kernel: Make AnonymousVMObject physical page APIs OOM safe
AnonymousVMObject::create_with_physical_page(s) can't be NonnullRefPtr
as it allocates internally. Fixing the API then surfaced an issue in
ScatterGatherList, where the code was attempting to create an
AnonymousVMObject in the constructor which will not be observable
during OOM.

Fix all of these issues and start propagating errors at the callers
of the AnonymousVMObject and ScatterGatherList APis.

SPDX License Identifiers are a more compact / standardized
way of representing file license information.

5d180d1f99 Everywhere: Rename ASSERT => VERIFY
(...and ASSERT_NOT_REACHED => VERIFY_NOT_REACHED)

Since all of these checks are done in release builds as well,
let's rename them to VERIFY to prevent confusion, as everyone is
used to assertions being compiled out in release.

We can introduce a new ASSERT macro that is specifically for debug
checks, but I'm doing this wholesale conversion first since we've
accumulated thousands of these already, and it's not immediately
obvious which ones are suitable for ASSERT.

ddd79fe2cf Kernel: Add WaitQueue::wait_forever and it use it for all infinite waits.
In preparation for marking BlockingResult [[nodiscard]], there are a few
places that perform infinite waits, which we never observe the result of
the wait. Instead of suppressing them, add an alternate function which
returns void when performing and infinite wait.

19d3f8cab7 Kernel+LibC: Turn errno codes into a strongly typed enum
..and allow implicit creation of KResult and KResultOr from ErrnoCode.
This means that kernel functions that return those types can finally
do "return EINVAL;" and it will just work.

There's a handful of functions that still deal with signed integers
that should be converted to return KResults.

e6f907a155 AK: Simplify constructors and conversions from nullptr_t
Problem:
- Many constructors are defined as `{}` rather than using the ` =
  default` compiler-provided constructor.
- Some types provide an implicit conversion operator from `nullptr_t`
  instead of requiring the caller to default construct. This violates
  the C++ Core Guidelines suggestion to declare single-argument
  constructors explicit
  (https://isocpp.github.io/CppCoreGuidelines/CppCoreGuidelines#c46-by-default-declare-single-argument-constructors-explicit).

Solution:
- Change default constructors to use the compiler-provided default
  constructor.
- Remove implicit conversion operators from `nullptr_t` and change
  usage to enforce type consistency without conversion.

da5cc34ebb Kernel: Fix some issues related to fixes and block conditions
Fix some problems with join blocks where the joining thread block
condition was added twice, which lead to a crash when trying to
unblock that condition a second time.

Deferred block condition evaluation by File objects were also not
properly keeping the File object alive, which lead to some random
crashes and corruption problems.

Other problems were caused by the fact that the Queued state didn't
handle signals/interruptions consistently. To solve these issues we
remove this state entirely, along with Thread::wait_on and change
the WaitQueue into a BlockCondition instead.

Also, deliver signals even if there isn't going to be a context switch
to another thread.

Fixes #4336 and #4330

68b361bd21 Kernel: Return ENOMEM in more places
There are plenty of places in the kernel that aren't
checking if they actually got their allocation.

This fixes some of them, but definitely not all.

Also, let's make find_one_free_page() return nullptr
if it doesn't get a free index. This stops the kernel
crashing when out of memory and allows memory purging
to take place again.

c8d9f1b9c9 Kernel: Make copy_to/from_user safe and remove unnecessary checks
Since the CPU already does almost all necessary validation steps
for us, we don't really need to attempt to do this. Doing it
ourselves doesn't really work very reliably, because we'd have to
account for other processors modifying virtual memory, and we'd
have to account for e.g. pages not being able to be allocated
due to insufficient resources.

So change the copy_to/from_user (and associated helper functions)
to use the new safe_memcpy, which will return whether it succeeded
or not. The only manual validation step needed (which the CPU
can't perform for us) is making sure the pointers provided by user
mode aren't pointing to kernel mappings.

To make it easier to read/write from/to either kernel or user mode
data add the UserOrKernelBuffer helper class, which will internally
either use copy_from/to_user or directly memcpy, or pass the data
through directly using a temporary buffer on the stack.

Last but not least we need to keep syscall params trivial as we
need to copy them from/to user mode using copy_from/to_user.

d89582880e Kernel: Switch singletons to use new Singleton class
MemoryManager cannot use the Singleton class because
MemoryManager::initialize is called before the global constructors
are run. That caused the Singleton to be re-initialized, causing
it to create another MemoryManager instance.

7a3ab6c517 Kernel: Make File::write() and File::read() return KResultOr<size_t>
Instead of returning a ssize_t where negative values mean error,
we now return KResultOr<size_t> and use the error state to report
errors exclusively.

788b2d64c6 Kernel: Require a reason to be passed to Thread::wait_on
The Lock class still permits no reason, but for everything else
require a reason to be passed to Thread::wait_on. This makes it
easier to diagnose why a Thread is in Queued state.

ab40cc60d1 Kernel: Fix glitched audio output in SB16 driver
We were not setting the DMA transfer mode correctly. I have absolutely
no clue how this could ever have worked, but it did work for months
until it suddenly didn't.

Anyways, this fixes that. The sound is still a little bit glitchy and
that could probably be fixed by using the SB16's auto-initialized mode.

9c856811b2 Kernel: Add Region helpers for accessing underlying physical pages
Since a Region is basically a view into a potentially larger VMObject,
it was always necessary to include the Region starting offset when
accessing its underlying physical pages.

Until now, you had to do that manually, but this patch adds a simple
Region::physical_page() for read-only access and a physical_page_slot()
when you want a mutable reference to the RefPtr<PhysicalPage> itself.

A lot of code is simplified by making use of this.

7d862dd5fc AK: Reduce header dependency graph of String.h

a3fa40fc07 Kernel: Enable IRQs before sending commands to devices
Without this fix, a very fast IRQ can preempt the enable_irq() call,
leaving that IRQ being unhandled.

0fc60e41dd Kernel: Use klog() instead of kprintf()
Also, duplicate data in dbg() and klog() calls were removed.
In addition, leakage of virtual address to kernel log is prevented.
This is done by replacing kprintf() calls to dbg() calls with the
leaked data instead.
Also, other kprintf() calls were replaced with klog().
