Remove the cheesy block cache from DiskBackedFS.

This should be reimplemented with proper paging support. This approach was
mostly just chewing kmalloc memory.

Fix broken SpinLock.
...
The SpinLock was all backwards and didn't actually work. Fixing it exposed
how wrong most of the locking here is.
I need to come up with a better granularity here.

Add a very naive block cache to the DiskBackedFileSystem.
...
This would be a lot better as an LRU. Right now it's a 32-slot
hash table with random eviction.

BlockDevice -> DiskDevice.
...
BlockDevice was the wrong name for this abstraction, since a block device
is a type of file in a unix system, and we should use that name for that
concept in the fs implementation.
